Request for reimbursement to another person

The form « Déclaration autre bénéficiaire » is to be used if you wish to:

• obtain reimbursements on your own account previously registered with the CMFEP
• if you have paid costs upfront for another person (other than your co-insureds)
• for yourself (if you are co-insured)
• change a prior declaration

IMPORTANT: If the CMFEP does not yet know your bank details or if you want to change them, please use the procedure "Bank account change" here. (lien myguichet CTIE à venir)
